All patients are asked if they have or wish to make an advance directive. If
you have an Advance Directive such as a Living Will, Health Care Proxy, or
Durable Power of Attorney for Health Care, please bring a copy of this with you.
If you need help to write an Advanced Directive, we can provide you with
information and assistance. Ask your nurse to contact the nursing supervisor for
assistance. Regional West uses the "Five Wishes" booklet.
